The following documents MUST be returned with your application.

1. Birth Certificate or other such document for proof of age. (State certified copy preferred)

2. High School Transcript (Diploma is not acceptable) or high school equivalency (GED) certificate and the official report of test results.

• Military Transfer or Discharge Form DD214 if applicable.

3. Document verifying any previous training or work experience. (Certificates of completion or Transcripts. Letters from previous Employers etc.)

4. Acknowledgement of substance abuse policy and authorization to release drug/alcohol testing information. (this is enclosed with the application, please read the Substance Policy for Tulsa Pipe Trades Training School FIRST. Sign and date the enclosed form.)

5. Enclose a letter of request if you are requesting credit for previous Training or Work Experience. A sample copy is contained in the information packet for preparing an application, which is enclosed also.

Please return the completed application as soon as possible. Incomplete applications will not be processed.
